Transcripts

The Florida College of Integrative Medicine (FCIM) issues official transcripts of academic performance only upon written authorization from the individual student. Each graduate receives two official transcripts at no cost if financial obligations are current. Subsequent transcripts cost $10.00 per transcript. The written request for transcript must include the following:

• Name when enrolled
• Current address
• Social security number
• Date of graduation
• Period of attendance
• Name and address of the individual or organization designated to receive the transcript
• Signature of student

A transcript request is not honored if the student has any outstanding financial obligations.

A transcript may take up to two weeks to process.
